Job summary
Armor Health in Columbus, OH is seeking a Discharge Planner/LPN Night for 2x12 shifts on a part-time basis at Franklin County Jail. You will coordinate discharge planning, consult with medical and behavioral health providers, and support transition programs for seriously ill patients.
The role requires a minimum of two years in correctional health or related fields, nursing or social/healthcare degree, and strong communication and computer skills.
Qualifications
- Minimum of two years’ experience in correctional health or criminal justice environment as case manager, discharge planner, nurse performing similar tasks
- Excellent written and verbal communication skills
- Excellent computer skills
- Familiar with health records, medical terminology, and health information services
Responsibilities
- Monitor chronic care logs and caseload to supplement referrals by staff
- Develop discharge planning referral criteria for intake medical screeners
- Consult with medical providers on current caseload to identify patients meeting criteria for pre-release health planning
- Consult with Armor behavioral health provider on patients with complex needs requiring coordination
- Participate in multidisciplinary planning for specialized transitional programs
