Qualifications
- Minimum of two years’ experience in a correctional health or related setting as case manager or discharge planner.
- Bachelor's degree or nursing degree required.
- Excellent written and verbal communication skills; familiarity with health records and medical terminology.
Responsibilities
- Monitors chronic care logs and caseload to supplement referrals by staff.
- Develops discharge planning referral criteria for intake medical screeners.
- Consults with medical providers on current caseload to identify patients for pre-release health planning.
- Consults with behavioral health providers on complex cases requiring coordination at release.
- Participates in multidisciplinary planning for transitional programs for seriously ill patients.
