Armor Health is hiring a Discharge Planner/LPN for 7a-7p night shifts at Franklin County Jail in Columbus, OH. You will coordinate pre-release care and referrals to ensure smooth transitions from custody.

Responsibilities include monitoring chronic care logs, setting discharge criteria, and collaborating with medical and behavioral health teams to support complex cases during release. Competitive benefits available.

Competenze

• Minimum of two years’ experience in a correctional health or related setting as case manager or discharge planner.
• Bachelor's degree or nursing degree required.
• Excellent written and verbal communication skills; familiarity with health records and medical terminology.

Responsibilities

Monitors chronic care logs and caseload as needed to supplement referrals by staff.

Develops discharge planning referral criteria for intake medical screeners.

Consults routinely with medical providers on current caseload to identify patients meeting criteria for receiving pre-release health planning.

Consults with the Armor behavioral health provider (including psychiatrist and licensed mental health professional)s on specific patients with complex needs that require supplemental coordination in addition to individual treatment plans or behavioral health referral plans upon release from custody.

Participates in multidisciplinary planning in support specialized transitional programs dedicated to seriously or chronically ill patients (such as community based mental health teams and HIV Jail Linkage programs).

Qualifications

• Minimum of two years’ experience, preferably in correctional health or criminal justice environment as case manager;discharge planner;nurse performing similar tasks related to referral, case management, and/or care coordination;or mental health related service coordinator


• Bachelors’ Degree in social sciences, human services, or health care related field or Nursing Degree


• Excellent written and verbal communication skills; excellent computer skills


• Familiar with health records, medical terminology, and health information services
