Job summary

Care Initiatives is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing teams in Iowa. This role supports comprehensive resident care, including assessments, medication administration, wound care, IV therapy where allowed, and caregiver supervision within a collaborative interdisciplinary team.

The LPN will lead on-unit nursing care, ensure safety, document accurately, and mentor CNAs to sustain high-quality resident outcomes. A valid Iowa LPN license and CPR are required.

Qualifications

• Valid LPN license in good standing (Iowa).
• Current CPR Certification.
• Documentation of State-approved LPN Supervisory Course or ability to obtain within first 90 days.
• Knowledge of federal and state long-term care regulations.
• Empathy, respect, patience and discretion in care.

Responsibilities

• Conduct ongoing assessments of residents' clinical status, including vital signs and changes.
• Deliver and oversee direct nursing care (wound care, IV therapy where allowed, catheter care).
• Administer medications and treatments with safe documentation.
• Serve as clinical leader on the unit, coordinating resident care and communicating with physicians, leadership, residents, and families.
• Supervise and mentor Certified Nursing Assistants and other staff.
• Ensure adherence to regulations and infection prevention practices.
• Provide education and emotional support to residents and families.
