Transitions Hospice in Springfield, IL is seeking a Registered Nurse for the after-hours night shift to provide compassionate hospice care overnight. You’ll assess and treat patients, monitor symptoms, and coordinate with the care team to ensure comfort and safety during the night.

As a Night Nurse, you’ll work with families who need reassurance and clear communication after hours, delivering high-quality nursing care and documentation in a fast-paced, quiet-hours environment.

Competenze

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.

Mansioni

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed for overnight patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health data and respond with clinical judgment.
• Record accurate clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support continuity of care.
• Ensure equipment and services are in place for patients.
• Maintain and grow your clinical skills through continuing education.
