Hospice of the East Bay in Pleasant Hill, CA is seeking a Registered Nurse to provide direct home-based nursing care and coordinate related support for patients and families.

Under the supervision of physicians and the interdisciplinary team, you will develop pain management plans, educate families, perform assessments, and document patient progress. Evening/NOC shifts required; valid CA RN license and CPR are required, with mileage reimbursement and benefits offered.

Qualifications

• Must be able to perform a complete nursing physical assessment and develop a care plan.
• Provide patient and family education and support.
• Document patient status, goals, care provided and outcomes in timely manner.

Responsabilités

• Assess patient pain and develop individualized pain management plan.
• Identify and address comfort care needs and symptoms.
• Coordinate patient and family services with interdisciplinary team.
• Document progress and update care plans at regular intervals (≤15 days).
• Support bereavement and community resources as needed.
