AccentCare Inc in Rhode Island is seeking a full-time Hospice IPC RN to join our inpatient facility team in Warwick. You will assess and coordinate comprehensive hospice care for patients and families, collaborating with physicians and the care team to ensure compassionate, high-quality service.

Must be a licensed RN with hospice/home health experience; CPR certification; willingness to be on-call. We offer a supportive, purpose‑driven workplace with benefits and professional development.

Qualifications

• Licensed RN in the state of practice; associate degree preferred.
• CPR certification required.
• One year of hospice/home health or related care experience preferred.

Responsibilities

• Assess, plan, implement and evaluate individualized hospice care plans.
• Coordinate with physicians and the interdisciplinary team to ensure compassionate care.
• Maintain accurate and timely documentation of patient records and care interventions.
• Educate patients and caregivers on medications, mobility and end-of-life preparations.
• Provide emotional and physical support during end-of-life care and participate in 24/6 coverage with the hospice team.
• Attend team meetings and adhere to nursing regulations.
