Job summary

Mercy is seeking a clinically driven Clinical Pharmacist (Hospital) for the night shift. You will work with physicians, nurses, and staff to optimize medication therapy, provide expert drug information, and support patient safety across the organization.

As a key resource, you will review orders, monitor therapies, and educate staff while advancing your clinical skills through ongoing development and interdisciplinary collaboration.

Qualifications

• Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy.
• Current Pharmacist License in the state of practice.
• Hospital or clinical pharmacy experience preferred.
• Strong clinical judgment and decision-making abilities.
• Excellent communication and collaboration skills.

Responsibilities

• Review patient records and medication orders to ensure safe, appropriate, and effective drug therapy.
• Evaluate patient conditions and identify opportunities to optimize medication treatment plans.
• Monitor medication therapies and intervene when necessary to improve outcomes and reduce risk.
• Assess treatment effectiveness and recommend therapeutic adjustments.
• Provide medication consultation to physicians, nurses, and patients.
• Advise on medication dosing, administration, contraindications, and potential drug interactions.
• Practice Medication Therapy Services through comprehensive patient assessment and evidence-based recommendations.
• Collaborate with providers to clarify orders and support optimal treatment decisions.
