Qualifications

Responsibilities

• Formulate and revise care plans based on patient response and evaluate effectiveness.
• Collaborate with interdisciplinary team to meet patient needs and goals.
• Administer medications and treatments per hospital policy.
• Delegate tasks to non-licensed staff according to standards of practice.
• Provide pre- and post-operative care.
• Educate patients and families on care and treatments.
• Facilitate discharge planning with care team.
