Job summary

Akron Children's Hospital Akron Main NICU is seeking a Part Time Case Manager for 24 hours per week, 8:00 a.m. to 4:30 p.m., onsite. The role manages hospitalization from pre-admission through one week post-discharge and requires BSN and RN licensure, with 3–5 years of experience.

Responsibilities include coordinating care with the health care team, completing education duties, leadership, QA/PI, and strong communication. BLS is required; CCM/CCCTM preferred.

Qualifications

• Bachelor's degree in Nursing (BSN) required; Master's preferred.
• Registered Nurse licensure required.
• 3–5 years of relevant experience.
• Current Health Care Provider BLS training from the American Heart Association required.
• CCM or CCCTM Certification preferred.

Responsibilities

• Assumes accountability for managing hospitalization of a caseload from pre-admission through one week post-discharge.
• Facilitates coordination of care with all health care team members to achieve outcomes.
• Completes educational responsibilities.
• Performs leadership duties.
• Performs quality assurance and performance improvement duties.
• Performs communication and networking duties.
• Performs systems-based practice duties.
• Addresses physical, psychosocial, educational, and safety needs of patients.
• Other duties as required.
