Job summary

Akron Main NICU in Ohio is seeking a Part-Time Case Manager to oversee hospitalization management for a caseload from pre-admission through one week post-discharge. You will coordinate care with the health care team, ensure educational responsibilities are met, and perform leadership and quality improvement duties.

The role requires 3–5 years of case management experience, RN licensure, BSN, and proficiency with MS Office and Epic EMR. Part-time 24 hours/week, onsite schedule.

Qualifications

• Bachelor's degree in Nursing is required.
• Registered Nurse licensure is required.
• Experience in case management is preferred.
• Proficiency in MS Office is required; Epic EMR experience preferred.

Responsibilities

• Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge.
• Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.
• Completes educational responsibilities.
• Performs leadership duties.
• Performs quality assurance and performance improvement duties.
• Performs communication and networking duties.
• Performs systems based practice duties.
• Demonstrates the knowledge and skills necessary to provide care for the physical, psych/social, educational, and safety needs of the patients served regardless of age.
• Other duties as required.
