Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, adapting plans as needed. This role serves as a clinical resource and role model for staff and students, with a sign-on bonus offered.

The ideal candidate will design comprehensive care plans, educate families and patients, and document per policy. Responsibilities include safe patient transfers, ongoing assessment, and collaboration with the medical team to ensure optimal outcomes.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) per IDFPR requirements.
• Basic Life Support (BLS) within 30 days (AHA).
• Completion of required continuing education for acute stroke care.

Responsibilities

• Transfers with assistance, proper body mechanics to ensure safety.
• Treats patients with dignity and empathy, engaging those in need of services.
• Designs and adjusts a care plan with goals, outcomes, and discharge planning.
• Educates patients, families, and staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Documents in accordance with CTSCP67 and regulatory standards.
• Screens patients to determine therapy needs and progression.
• Supervises PTA and rehab technicians per regulatory guidelines.
