Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses.
• Performs examination using relevant methods and history.
• Produces documentation in accordance with CTSCP67.
• Screens patients to determine need for therapy services.
• Supervises PTA and rehabilitation technicians as needed.
• Communicates progress and discharge recommendations to the medical team.
