Qualifications

Bachelor's degree in Physical Therapy is required.
Licensed Physical Therapist in good standing; IL license referenced in posting.
BLS certification within 30 days of hire.
Completion of required continuing education for acute stroke care.

Responsibilities

Transfers patients safely using proper body mechanics.
Treat patients and colleagues with dignity, empathy and respect.
Design and adjust care plans with goals, outcomes and discharge planning.
Educate patients, families and staff using effective methods.
Evaluate clinical findings to support PT diagnoses and outcomes.
Conduct examinations using reliable methods aligned with chief complaint.
Document according to CTSCP67 policy.
Screen patients to determine need for additional therapy services.
Coordinate with supervision of PTA and rehab technicians.
Communicate patient status, discharge needs and equipment requirements to medical team.
