Job summary

UCI Health is seeking a Bridges Coordinator to oversee newborn family screening and risk assessment at the assigned facility, using the Bridges Maternal Child Health Screening Tool to identify families who may need support services. The incumbent provides nonclinical bedside education and guides families to community resources for breastfeeding, immunizations, newborn care and postpartum wellbeing.

Qualifications

• Strong interpersonal communication and cultural responsiveness.

Responsibilities

• Coordinate newborn family screening and risk assessment using Bridges tool.
• Ensure prescreening, eligibility, and referrals to enhanced services and home visiting.
• Educate families on community resources for breastfeeding and newborn care.
• Manage materials and ensure data privacy and HIPAA compliance.
• Liaise with First 5 Orange County / Bridges leadership and program staff.
• Support program performance metrics and contract compliance.
