Job summary

Central Jersey Family Health Consortium, Inc. is seeking a Program Support Specialist for the Family Connects home visiting program. The role connects newborn families to services, including those affected by infant loss, and data-entries the countywide eligible birth population.

The PSS follows up with families after nurse visits to assess satisfaction and nurse referral outcomes, coordinates nurse scheduling, and supports outreach with hospital birth centers to recruit families.

Qualifications

• Education: Associates degree required; Bachelor’s degree preferred in education or health related field.
• Experience in non-profit, maternal child health and education is strongly preferred.
• Experience in healthcare delivery or social service organization and/or community interfacing preferred.
• Verbal and written fluency in English and Spanish; strong data entry and attention to detail.
• Collaborative team player with ability to work independently to meet deadlines.
• Experience serving underserved or high-need populations with cultural sensitivity and strong interpersonal skills.

Responsibilities

• Coordinate scheduling of all nurse assignments and manage the client database.
• Assist with client recruitment and establish contact with families to recruit for nurse visits.
• Build relationships with hospital birth centers and staff to support recruitment and education.
• Schedule nursing visits and conduct post-visit follow-ups; track services and referrals as needed.
• Handle appointment confirmations via text messaging and reschedule when necessary.
• Participate in staff meetings and trainings; perform other duties as assigned by program leadership.
