Job summary

Reliant Medical Group, part of the Optum family, is seeking a Nurse Practitioner or Physician Assistant to join its New Patient Intake team in Worcester. This role focuses on improving access for new patients, conducting initial evaluations, coordinating care, and helping patients transition to long-term primary care providers.

The position offers a Monday-Friday daytime schedule with no on-call, weekends, or holidays, in a collaborative environment within Adult and Family Medicine.。

Qualifications

• Board Certified NP or PA required.
• Massachusetts licensure in good standing.
• Active DEA registration is required.
• Minimum 3 years of APC experience.

Responsibilities

• Perform new patient intake and act as the first clinical contact for Adult and Family Medicine.
• Evaluate acute and chronic conditions and initiate preventive care.
• Coordinate care and assist patients in transitioning to long-term primary care providers.
