Job summary

Reliant Medical Group, part of the Optum family, in Worcester, MA, seeks a Nurse Practitioner or Physician Assistant for its New Patient Intake team. The role focuses on improving access, conducting initial evaluations, coordinating care, and transitioning patients to long-term primary care.

The position offers a daytime Mon-Fri schedule with 14–18 patient days, 20–30 minute visits, and a strong emphasis on care coordination within a collaborative team.

Qualifications

• Board Certified as NP or PA.
• Unrestricted Massachusetts license.
• Active DEA license.
• Minimum 3 years as APC.

Responsibilities

• Handle new patient intake and improve access to primary care.
• Be the first clinical contact for Adult and Family Medicine patients.
• Evaluate acute and chronic conditions and initiate preventive care.
• Collaborate with physicians, APCs, nurses and staff in a team-based setting.
• Manage a patient load of 14–18 per day with 20–30 minute visits.
