Reliant Medical Group, part of the Optum family, in Worcester, MA, seeks a Nurse Practitioner or Physician Assistant for the New Patient Intake team. This role focuses on improving access for new patients, conducting initial evaluations, coordinating care, and guiding patients to long-term primary care.

Mon-Fri daytime schedule, no call, weekends, or holidays. Expected workload 14–18 patients per day with 20–30 minute visits. Competitive benefits with OptumCare support.

Qualifications

• Board Certified as a NP or PA.
• Unrestricted licensure in Massachusetts.
• Active DEA license.
• Minimum 3 years of APC experience.

Responsabilités

• New patient intake and access-focused primary care responsibilities.
• Evaluate acute and chronic conditions and initiate preventive care.
• Coordinate care and help patients transition to long-term primary care.
• Serve as the first clinical contact for Adult and Family Medicine patients.
