Job summary

The Ridge RTC in New Hampshire is seeking a Case Manager to join the Altior network, coordinating with clinical staff to support residential clients and families.

Responsibilities include assessment, care planning, discharge planning, education on resources, and documenting all clinical contacts in the EMR. A Bachelor's degree and 2+ years of experience are preferred. Competitive hourly wage and benefits offered.

Qualifications

• Bachelor's degree in social work or related field preferred.
• Minimum 2 years of experience in residential/therapeutic setting.
• Strong leadership, communication, and organizational skills.
• Crisis management and conflict resolution abilities.

Responsibilities

• Provide comprehensive case management, including assessment, care planning, and ongoing support.
• Participate in the interdisciplinary treatment planning process and perform proactive discharge planning.
• Provide education to clients and their families regarding community resources, benefits, and coverage.
• Coordinate and facilitate meetings between clients, families, residential care team.
• Facilitate group sessions and support recovery-focused programming.
• Document all clinical contacts and documentation through EMR; maintain confidentiality.
