Job summary

The Ridge RTC in New Hampshire is seeking a Case Manager to join the Altior network, coordinating with clinical staff to support residential clients and families.

Responsibilities include assessment, care planning, discharge planning, education on resources, and documenting all clinical contacts in the EMR. A Bachelor's degree and 2+ years of experience are preferred. Competitive hourly wage and benefits offered.

• Participate in the interdisciplinary treatment planning process and perform proactive discharge planning.
• Provide education to clients and their families regarding community resources, benefits, and coverage.
• Coordinate and facilitate meetings between clients, families, residential care team.
• Facilitate group sessions and support recovery-focused programming.
• Document all clinical contacts and documentation through EMR; maintain confidentiality.

Our family of services comprises three distinct mental health treatment programs, including a specialized program for US Veterans. With over 15 unique locations, we manage and support 500+ dedicated employees serving over 300 residential clients daily located across five states: California, Idaho, Maine, New Hampshire, and Texas.

As the Ridge Case Manager ,you willcollaboratewith other members of the clinical team,toserveas a liaison for the treatment team, client,familiesandaftercare services.

Key Responsibilities:

• Provide comprehensive case management, including assessment, care planning, and ongoing support.

• Support a safe, structured, and trauma-informed residential environment.
• Facilitate and providetimelydocumentation.
• Adhering to company policies and proceduresregardingconfidentiality and privacy.
• Other duties may be assigned.

Qualifications:

• Bachelor's degree in social work, psychology, healthcare administration, ora relatedfield (preferred).
• Minimum2years of experience in a residential or therapeutic setting.
• Leadership skills that foster a culture of inclusivity and excellence.
• Excellent communication, leadership, and organizational skills.
• Strong crisis management and conflict resolution skills.

Salary Range

$28 - $30 USD
