Job summary

JobRx, Inc. is seeking an MTM Pharmacist to work within a clinic-based care team, performing comprehensive medication reviews and coordinating resolutions with an interdisciplinary group to optimize patient outcomes.

You will educate patients, collaborate under CPAs, monitor labs, and contribute to quality improvement while ensuring regulatory compliance and patient-centered care across UPH geographies in Iowa.

Qualifications

• PharmD required.
• Board Certification in Ambulatory Care or Geriatrics preferred.
• 3–5 years pharmacist practice experience or PGY1/PGY2 residency.
• Current pharmacist license in the state where MTM services are provided; ability to obtain licensure in additional UnityPoint Health states.

Responsibilities

• Provide comprehensive medication reviews, reconciliation, and patient education to optimize medication therapy outcomes.
• Collaborate with providers to adjust therapies under Collaborative Practice Agreements and recommend evidence‑based treatment plans.
• Order and monitor appropriate laboratory tests to support safe and effective medication use.
• Assist patients with medication affordability and adherence strategies.
• Lead and participate in quality improvement initiatives, workflow enhancements, and standards‑of‑care development.
• Educate clinic staff and interdisciplinary teams on medication‑related best practices.
• Maintain compliance with regulatory requirements while demonstrating a commitment to exceptional patient‑centered care.

Overview

Working as a member of the clinic-based care team, the MTM Pharmacist proactively and collaboratively identifies and coordinates the resolution of medication-related issues based on an interdisciplinary, patient‑specific care plan. Working at the top of their license and taking a comprehensive approach, the pharmacist helps patients achieve their best possible outcomes from their medication therapies. The pharmacist also serves as the medication expert to raise the overall knowledge of the care team and improve workflows where possible.

Strong preference for candidates residing within the UPH geographies of Iowa, Illinois, & Wisconsin.

Qualifications

Education

• Doctor of Pharmacy (PharmD) required.
• Board Certification in Ambulatory Care, Geriatrics, or a related specialty preferred.

Experience

• 3-5 years of pharmacist practice experience, or completion of a PGY1/PGY2 pharmacy residency.

Licensure & Certifications

• Current pharmacist license in the state where MTM services are provided.
• Ability to obtain and maintain pharmacist licensure in additional UnityPoint Health service area states as needed.
