Job summary

Qualifications

• Masters Degree in Social Work from an accredited institution.
• 1–2 years working within Healthcare or Hospital preferred.
• Licensed through the State of Ohio Counselor, Social Worker and Marriage and Family Therapist Board, LSW required.

Responsibilities

• Collaborate with RN Case Manager and Integrated Care staff for transition of care and discharge planning.
• Provide psychosocial assessment and intervention to help families cope with illness and disability.
• Educate healthcare professionals and community on psychosocial factors affecting care.
• Support onboarding and training for new staff and students within the department.
