Methodist Le Bonheur Healthcare in Memphis seeks an experienced Registered Nurse to serve as a consistent care coordinator across the continuum of care within our health system. You will assess physical, psychological, and social needs and coordinate communication with providers to improve the patient and family care experience.

This role involves nursing care in clinical/office settings, documenting patient data, and supporting education, screening, prevention, and follow-up.

Qualificações

• Required - Associates Degree Nursing.
• Preferred - Bachelor's Degree Nursing.
• Registered Nurse 0-1 years experience required.
• Clinical nursing 1-3 years preferred.

Responsabilidades

• Assess the physical, psychological, and social needs of the patient, and improve the care experience for the patient and their family.
• Create a seamless flow through the health system to facilitate the patient’s care.
• Develop and coordinate education, resources, and activities that provide understanding and support for patients and families.
• Maintains reliable systems to document, track and monitor patient data.
• Performs clinical procedures within the scope of nursing practice and clinical training.
• May perform telephone duties within the scope of RN role.
• Performs other job functions as assigned or requested.
