Utilize Health seeks a Care Manager to coordinate care for members in East Tennessee, conducting community and home visits to support self-management and reduce complications. You will navigate healthcare systems, communicate with providers, and document activities in the EMR to maintain continuity of care.

The role involves travel across East Tennessee, collaboration with primary care and specialists, and educating patients and families on self-care strategies to achieve health goals.

Qualifications

• Bachelor’s or Master’s degree in a healthcare or behavioral health program, or a bachelor’s with 2+ years in care management or related field.
• Field-based position traveling within East Tennessee, primarily Metro Knoxville.
• Caseload management with on-site visits and travel to Knox, Hamblen, Sevier, Jefferson counties and surrounding zip codes.

Responsabilités

• Serve as primary representative of Utilize Health to members and advocate for their care needs.
• Manage a member caseload through in-person, phone, and electronic communications.
• Conduct in-person and phone visits to complete screenings and identify needs for self-management and complication prevention.
• Develop individualized care plans with measurable goals and interventions.
• Ensure timely and accurate documentation in the EMR.
• Coordinate care with providers and community resources to close gaps in care.
• Coach members on self-care to improve personal and clinical outcomes.
• Schedule and accompany members to appointments as needed.
