Job summary

SSM Health Rehabilitation Hospital - Neuro Transitional Center in O’Fallon, MO is seeking a Licensed Practical Nurse (LPN) to join our PRN Night shift team. This role involves supervising Neuro Rehabilitation Coaches, delivering direct patient care, and supporting treatment plans to promote patient safety and independence.

The LPN will participate in nurse delegation, handle medication administration for up to 12 patients, and collaborate with the neuro transitional rehab team.

Qualifications

• Current LPN license in good standing
• 3–5 years experience in rehab or similar setting
• Ability to supervise staff and coordinate care

Responsibilities

• Supervises Neuro Rehabilitation Coaches and delivers direct patient care
• Assists with treatment plans and ensures wellbeing and safety of all patients
• Provides education to patients, families and coworkers
• Coordinates staffing with leadership and ensures shifts run smoothly
• Assists patients in activities and promotes engagement in education, therapy and community settings
• Completes required documentation and ensures documentation per policy
