Mercy Rehabilitation Hospital - Springfield is seeking a Licensed Practical Nurse (LPN) to join our FT Nights team. You will provide direct and indirect patient care using the nursing process, collaborate with the care team to design personalized plans, and monitor patient conditions for timely physician communication.

Qualifications

• Graduation from an accredited BSN, ADN, or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse (LPN).
• CPR/BCLS certification; ACLS preferred.
• Minimum six months' Medical/Surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.

Responsibilities

• Deliver direct and indirect patient care using the nursing process—Assessment, Planning, Intervention, and Evaluation.
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
• Play a key role in the discharge planning process to support safe, successful transitions.
