Rehab Without Walls Neuro Rehabilitation in Austin, TX is seeking a Rehab Case Manager for PT/OT/SLP/LCSW licensure. You will lead a transdisciplinary team to coordinate comprehensive patient care and community reintegration, acting as the central contact for patients, families, and providers.

The role requires a TX therapy license and experience in neuro-rehabilitation. You will collaborate with clinicians, payers, and care partners to develop and monitor treatment plans and ensure timely

Qualifications

  • Current Therapeutic License in state of practice (OT, PT, SLP, LCSW, LPC, NP, Rehabilitation Counseling/Psychology).
  • Maintains relevant professional licensure/registration in the state of practice.
  • Minimum of two-year's experience in a rehabilitation setting with neurologically impaired patients.

Responsibilities

  • Manages provision of individual patient services and collaborates with clinical staff to develop rehabilitation plans and coordinate services.
  • Formulates transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives, and service delivery with patient/care partner and clinical team.
  • Completes CC evaluation and develops patient initial, progress, and discharge reports; monitors status and documents outcomes.
  • Monitors satisfaction of patient, family, physician, payer/case manager, and referral sources.
  • Leads interdisciplinary case conferences, communicates Treatment Plans/Progress Reports to physicians, external case managers/payors, and family members.
  • Schedules/intervenes in interdisciplinary case conferences with clinical team, payers, physicians, and family.
  • Communicates progress to patient/family, payer, and physician; addresses important issues as they arise.
  • Facilitates acquisition of necessary equipment to meet functional goals with funding source.
  • Participates in medical provider meetings to report/resolve medical issues.
  • Monitors hours of service and reports changes to local leadership.
  • Negotiates extensions of service with funding sources.
  • Obtains and tracks orders from physicians for ongoing services.
  • Adheres to Code of Conduct, HIPAA, compliance programs and applicable laws.
  • Attends staff development and continuing education as required.
  • Performs other duties as assigned.

Overview

Rehabilitation Case Manager (PT/OT/SLP/LCSW) Austin, TX

  • This role is multi-faceted, and supports a more efficient, effective and comprehensive transdisciplinary neuro-rehabilitation program where you'll make a meaningful impact on patients' recovery journeys, similar to case managers utilized within inpatient rehab hospitals.
  • Act as the team lead and central point of contact to coordinate, facilitate, and orchestrate comprehensive patient care, ensuring that rehabilitation and community reintegration needs are successfully met, positioning you as a key driver of patient outcomes.
  • Requires therapy license in the state of TX (PT/OT/SLP/LCSW/LPC/NP)
Responsibilities
  • Manages the provision of individual patient services and collaborates with clinical staff to develop rehabilitation plans. Coordinates the ongoing provision of rehabilitation services.
  • Formulates comprehensive, transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives and service delivery for referred patients in conjunction with the patient/care partner and the clinical team.
  • Completes CC evaluation and develops patient initial, progress and discharge reports and monitors the patient's status on an ongoing basis. CCs are responsible for completion of patient clinical outcome tools based on diagnosis.
  • Monitors patient, family, physician and payer/case manager, referral source satisfaction with the program on a regular basis.
  • Leads interdisciplinary case conferences, team meetings, and communicates Treatment Plans/Progress Reports on a regular basis to the physicians, external case manager/payor and/or patient/family members.
  • Schedules/facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians and family members.
  • Communicates regularly with patient, family/ care partner, payer/case manager and physician regarding patient progress and any important issues that arise.
  • Facilitates acquisition of necessary equipment to enable achievement of agreed upon functional goals (i.e. HMO, insurance carrier, etc.) with funding source.
  • Participates in medical provider meetings to report/resolve medical issues for person served.
  • Monitors projected hours of service to be delivered to the patient and reports hours/subsequent changes to the local leadership (Executive Director/Director of Clinical Management/Program Manager).
  • Negotiates with funding sources for extensions of service beyond initial authorizations.
  • Acquires and tracks orders from physician for all provided ongoing services after the initial orders are obtained.
  • Adheres to standards of the Company's Code of Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
  • Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations. Maintains and enhances clinical practice skills.
  • Perform other duties, as assigned.
Qualifications
  • Current Therapeutic License in state of practice (OT, PT, SLP, LCSW, LPC, NP, Rehabilitation Counseling/Psychology)
  • Maintains relevant professional licensure/registration/certification in the state of practice.
  • Current certification as Certified Brain Injury Specialist (CBIS) preferred.
  • Current BLS (Basic Life Support) First Aid and CPR certifications.
  • Minimum of two-year's experience in a rehabilitation setting with neurologically impaired patients
  • Prior case management and/or insurance administration experience preferred.
  • Demonstrates excellent organization, problem solving, decision making and communication skills.
  • Organized and able to function under minimal supervision.
  • Has reliable transportation and able to travel to/from multiple assignments/locations on a daily basis.
