Job summary

ReMed, A Collage Rehabilitation Partner in Irwin, PA, is seeking a compassionate Direct Support Professional (DSP) for a part-time role with a Mon-Thu 6:30 AM - 3:30 PM schedule to support adults with brain injuries across Irwin or Greensburg.

You will assist with daily living activities, promote independence, monitor health and nutrition, support vocational and recreational pursuits, and maintain a safe, home-like environment while accurately documenting shifts, goal sheets, and incident

Qualifications

• Proof of high school diploma or equivalent is required.
• Valid driver's license with ability to travel up to 20% of the time.
• Ability to lift up to 50 lbs.

Responsibilities

• Promote independence: assist with ADLs, community living skills, and rehab goals.
• Health & Wellness: assist with medication administration, monitor nutrition, prepare meals.
• Proactive Behavioral Support: support clients in challenging situations and implement behavior plans.
• Observation & Advocacy: identify changes in clients' conditions and alert the team.
• Transportation & Community Activities: safely transport clients to appointments and outings.
• Compliance & Documentation: complete shift documentation per CARF/state/federal standards.
• Environment & Safety: maintain a clean, safe living environment.
• Team Communication: participate in hand-offs and collaborate with families and care teams.
