Job summary

ReMed, A Collage Rehabilitation Partner, is seeking an Outpatient Direct Support Professional (DSP) in Paoli, PA. This full-time role supports adults with brain injuries in home and community settings, implementing rehabilitation strategies to promote independence.

Responsibilities include assisting with ADLs, supporting health and wellness, coordinating transportation, and documenting care in compliance with CARF/state standards.

Qualifications

• Education: High school diploma or equivalent.
• Operational requirements: Valid driver’s license with ability to travel up to 20% of time.
• Physical demands: Lift up to 50 lbs.

Responsibilities

• Promote Independence: Assist with Activities of Daily Living (ADLs), community living skills, and rehabilitation goals.
• Health & Wellness: Assist with medication administration, monitor nutrition, and prepare meals.
• Proactive Behavioral Support: Support clients during challenging situations and follow behavior management plans.
• Observation & Advocacy: Identify changes in behavior, emotions, cognition or physical condition and alert the treatment team.
• Transportation & Community Outings: Safely transport clients to medical appointments and activities.
