Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a Physical Therapist (PT) for neuro home and community care. You will provide true one-on-one neuro rehab in patients' homes and communities, with control over your schedule and caseload.

This role offers a $1,000 sign-on bonus for select roles and a PRN weekend option with additional incentive pay. You’ll work with neurological conditions to help people regain mobility and independence in real environments.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support or CPR certification in accordance with state regulations.
• Minimum of one year’s work experience as a physical therapist preferred.
• Knowledge of rehabilitation techniques related to neurological injury.
• Ability to lift 50 pounds and move/transfer patients safely.
• Excellent verbal and written communication.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home and community safety recommendations.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Collaborate with other disciplines to enhance patient outcomes.
