Job summary

Advocate Health is seeking a licensed Physical Therapist for outpatient rehabilitation focusing on neurology. The role involves evaluating patients, establishing care plans, and coordinating discharge and home programs within a dynamic outpatient setting.

Responsibilities include documenting care, ensuring safety and infection control, and collaborating with physicians and staff to meet functional goals. A sign-on bonus of $5,000 is offered along with a 40-hour work week.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Licensed Physical Therapist in the state you work.
• BLS for Healthcare Providers within 90 days (if required) by American Heart Association.

Responsibilities

• Evaluate musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement individual treatment based on evaluation results and goals.
• Discharge planning with home programs, patient/family teaching, and transition planning within the continuum of care.
• Document patient care and progress per department policy, including evaluations, progress notes, and discharge summaries.
• Maintain safety and infection control measures; coordinate with stakeholders to achieve optimal patient care.
• Manage assigned caseload and assist with coverage based on department needs.
