Job summary

Cincinnati Children’s is seeking a Social Worker for Dialysis at Burnet Campus to join the Social Service team. The role supports psychosocial assessments, resource coordination, and counseling to families navigating chronic medical care.

Qualifications include an MSW and Ohio LSW license; experience is preferred but not required. Salary ranges from government‑set starting figures with comprehensive benefits beginning at day one; full-time, 40 hours per week.

Qualifications

• Master’s degree in Social Work required.
• Licensed Social Worker (LSW) in the State of Ohio required.
• Past experience preferred but not required.

Responsibilities

• Psychosocial Assessment—performs psychosocial assessments considering family, social, and medical history; maintain current knowledge of medical/mental health diagnoses; report suspicions of abuse; document accurately.
• Resource Support—collaborate with health care team to develop treatment plans and identify resources for patients and families; maintain knowledge of internal/external resources.
• Psychosocial Interventions—provide counseling and education to patients and families; support self-management for high-risk patients; coordinate and facilitate team meetings.
• Interdisciplinary Collaboration—advocate for patient/family needs within the health care system; serve as a liaison and mediator among teams.
• Collaboration—educate and partner with teams on psychosocial care and discharge planning.
