Job summary

Franciscan Health is seeking a full-time Board Certified/Board Eligible Nephrology Physician to join our Lafayette practice and dialysis outreach locations. Provider will divide time between clinic, rounding and dialysis outreach with minimal travel to Crawfordsville and Rensselaer.

Qualifications

• Doctorate - Required.
• Physician License (MD/DO) - Upon Hire Required.
• Board Certification in Specialty Discipline (or eligibility if appropriate) - Upon Hire Required.
• Basic Life Support (BLS) - Upon Hire Required.

• Coordinate and deliver the highest quality of patient care through collaboration.
