Job summary

Qualifications

Responsibilities

• Assess patients in the hemodialysis unit and initiate care plans.
• Coordinate with Nephrology team to follow up and adjust therapy.
• Document data, orders and interventions accurately in the medical record.
• Educate patients and families and support care planning.
• Communicate with outpatient dialysis units and ensure timely care.
