Job summary

Physician Consultants of Georgia / Midtown Vascular Center is seeking an experienced Nephrology Nurse Practitioner to join our clinical team. The successful candidate will manage inpatient rounds and outpatient visits, focusing on CKD, AKI, hypertension, and electrolyte disorders, with dialysis patient care as needed.

Applicants must hold an active NP license and national board certification, with nephrology experience preferred.

Qualifications

• Active NP license and national board certification are required.
• Nephrology experience preferred, with inpatient and outpatient management.
• Strong clinical judgment and communication skills are essential.
• Experience with hospital rounding and dialysis management is desirable.

Responsibilities

• Conduct hospital rounds and manage inpatient nephrology patients.
• Perform initial and follow-up in-office visits for nephrology patients.
• Evaluate and manage CKD, AKI, hypertension, electrolyte disorders, and other renal conditions.
• Manage patients requiring hemodialysis and peritoneal dialysis.
• Review labs, imaging, medications, and treatment plans; adjust as needed.
• Order appropriate lab tests and imaging within scope of practice.
• Document thoroughly in the electronic medical record.
• Coordinate care with nephrologists, hospitalists, dialysis facilities, and PCPs.
• Provide patient and family education on renal disease, dialysis, and diet.
• Support continuity of care between hospital discharge and outpatient follow-up.
