Job summary

Panoramic Health's Fall River Nephrology team is hiring a Nurse Practitioner to provide comprehensive CKD/ESRD care across outpatient clinics, dialysis centers, and hospital settings.

You will collaborate with nephrologists and dialysis teams to deliver coordinated, patient-centered care and contribute to a diverse clinical environment in southeastern Massachusetts. Strong clinical skills and MA NP licensure are required.

Qualifications

• Master's or Doctoral degree from an accredited NP program.
• Massachusetts NP license or eligibility for licensure.
• Current DEA registration or ability to obtain.
• Strong clinical assessment, diagnostic and patient management skills.
• Excellent communication and collaboration abilities.

Responsibilities

• Evaluate, diagnose, and manage CKD, ESRD and related disorders in outpatient nephrology.
• Develop individualized treatment plans to preserve kidney function.
• Monitor labs, diagnostics and medication therapies.
• Educate patients and families on disease management and options.
• Coordinate referrals and follow-up care with nephrology teams.
• Manage hemodialysis and peritoneal dialysis patients per guidelines.
• Assist inpatient nephrology management and care transitions.
• Collaborate with physicians, nurses, and care managers to ensure continuity.
