Summary

This role involves implementing policies, programs, and activities. The primary responsibility is performing psychosocial assessments on families' ability to meet their child's healthcare needs. It includes ensuring proper documentation per accreditation standards and organizational policies. This hourly position is similar to exempt role 2054 and is typically used for part-time or PRN assignments.

Responsibilities
  1. Perform biopsychosocial screening and assessments of families' ability to meet healthcare needs, applying specialized knowledge and advanced practice skills.
  2. Facilitate parent/child adjustment to illness and hospitalization.
  3. Coordinate anticipatory discharge planning to ensure home care is arranged prior to hospital discharge.
  4. Lead multidisciplinary patient care conferences focusing on medical and biopsychosocial needs.
How You’ll Be Successful

Credentials

Work Experience
  • At least 2 years of previous social work experience (preferred)
  • At least 4 years of related experience (preferred)
Education
  • Graduate or professional degree, or equivalent experience (required)
Licenses and Certifications
  • State of Texas LMSW license (required)
  • State of Texas LCSW license (preferred)
