Job summary

Cooper University Health Care in Camden, NJ seeks Navigator Specialists to provide ongoing supportive services for patients with substance use disorders, including education, service coordination, linkage to MAT and behavioral health services.

You will deliver care-coordination and linkage activities within healthcare, social service, and justice systems, fostering trust and engagement in treatment while meeting patients in diverse clinical settings.

Qualifications

• RN, LPN, BA with 2 years lived experience, or A.A. with 4 years lived experience, or CMA with 4 years lived experience.
• Lived experience navigating the criminal-legal system preferred.
• Bilingual candidates preferred.
• High School Diploma REQUIRED.

Responsibilities

• Provide ongoing supportive services to patients with substance use disorders.
• Offer education, service coordination, linkage, and support across healthcare, social services and justice systems.
• Provide ongoing care-coordination and case management.
• Guide linkage to MAT, behavioral health treatment, and social services.
• Foster one-on-one supportive relationships and encourage engagement in treatment.
