Job summary

Cooper University Health Care in Camden, NJ seeks a Navigator Specialist in the Center for Healing to provide ongoing supportive services to patients with substance use disorders.

You will deliver education, service coordination, linkage to MAT, behavioral health treatment, and social services, guiding patients through appropriate clinical options.

This role emphasizes building trusting relationships, in diverse clinical settings, including ED and outpatient appointments.

Qualifications

• RN, LPN, or CMA with 2+ years lived experience
• BA with 2+ years lived experience or an Associate with 4+ years lived experience
• CMA with 4+ years lived experience
• High School Diploma REQUIRED
• 1 year of relevant experience if candidate has a BA or 2 years if candidate has an Associate or Certified Peer Recovery Specialist
• 4 years of relevant experience if candidate is CMA
• Lived experience navigating criminal-legal system preferred
• Bilingual candidates preferred

Responsibilities

• Provide ongoing care-coordination and case management.

Short Description

Navigator Specialists in the Center for Healing provide ongoing supportive services to patients with substance use disorders.

They provide education, service coordination, service linkage, and support to patients within the healthcare, social service, and justice systems.

Provide ongoing care-coordination and case management services.

Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services to individuals with Substance Use Disorder.

Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder

Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome

Provide continuity of care by meeting patients in various clinical care settings including the following:

• Emergency department visits, addiction treatment appointments, medical appointments, social service appointments, and therapeutic appointments.
• Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc.
• Perform outreach by meeting with patients in the community and act as allies and advocates for patient services
• Attend Addiction Medicine clinical and programmatic conferences and meetings as required

Experience Required

Registered Nurse, OR

Licensed Practical Nurse with 2 years lived experience, OR

Baccalaureate (BA) degree with 2 years lived experience, OR

Associate degree with 4 years of lived experience, OR

Certified Medical Assistant with 4 years of lived experience

Education Requirements

High School Diploma REQUIRED

1 year of relevant experience ifcandidatehasbachelor'sdegree inrelatedfield or 2 years of relevant experience ifcandidatehasassociate'sdegree or is a Certified Peer Recovery Specialist.

4 years of relevant experience, either clinical or lived, ifcandidateis a Certified Medical Assistant.

Lived experience with navigating the criminal-legal system preferred.
