• Coordinate care across pre-procedure, hospitalization, discharge and post-hospital follow-up.
• Provide leadership in care coordination for interventional cardiology and STEMI patients.
• Support implementation of assigned projects across cardiology services.
• Collaborate with care teams to improve patient outcomes.

The Interventional Cardiology Nurse Navigator serves as a consistent resource to support optimal care delivery for cardiology with a focus on interventional cardiology and STEMI patients as well as other projects as assigned This nurse navigator takes on a leadership role in developing the coordination of care during pre-procedure, hospitalization, discharge planning and facilitating post-hospitalization follow-up.

Required Qualifications:

• Bachelor's degree in nursing (BSN) from an accredited school
• Upon hire: Washington Registered Nurse License
• Upon hire: National Provider BLS - American Heart Association
• 3 years of experience as a registered nurse in cardiovascular services.
