Broadlawns Medical Center in Des Moines, IA is seeking a Patient Navigator to support patients and caregivers in accessing health care and community resources, and to guide them through the healthcare continuum.

Under the Clinic Manager's supervision, you will coordinate care with the care team, promote self-management, and ensure adherence to the plan of care, collaborating with physicians, hospital staff, and community resources.

Qualifications

• Bachelor’s degree in public health or related health service field.
• Knowledge of health care system and community resources available.
• Experience with data collection and reporting.
• General knowledge of computers and basic software.

Patient Navigator GENERAL DESCRIPTION

Under the supervision of the Clinic Manager, this position works to develop a supportive rapport and assist patients and their caregivers to access health care and community resources and navigate through the healthcare continuum. Functions as a member of the patient’s collaborative care team to coordinate care and address care plan preventative health and wellness goals. Works collaboratively with the patient to foster self-management and compliance to their clinical plan of care. Works collaboratively with physician, hospital, and community resources to support the patient's clinical plan of care.

M-F 8A-4:30P

• Completes all annual mandatory education training within defined time frame (safety training, corporate compliance, performance appraisals, confidentiality, etc.).

MINIMUM QUALIFICATIONS

• Bachelor’s degree in public health or related health service field.
• Knowledge of health care system and community resources available.
• Experience with data collection and reporting.
• General knowledge skills in the use of computers including basic computer applications.
• BLS certified for the healthcare provider or obtained within three months of hire.
