Job summary

Community Bridges, Inc. (CBI) seeks a Navigator II to enhance client success by engaging and connecting individuals with needed services and resources. The role includes outreach, engagement, case management, and ongoing wraparound supports within a multi-disciplinary team.

The Navigator II will document and track interactions according to AAC, ADHS/DBHS, CARF, and CBI policies, and map the next steps for accessing services to achieve ongoing success.

Qualifications

• Minimum 12 months of recovery from substance use and/or mental health disorders.
• 1-3 years of healthcare-related work experience preferred but not required.
• Minimum 90 days experience in a Navigator or Peer Support Specialist role.
• Current AZ driver's license in good standing.
• Must provide a 39-month MVR.
• Arizona Fingerprint Clearance card (program specific).
• High school diploma or GED required; associate degree preferred.

Responsibilities

• Conduct outreach, engagement, case management, and wraparound supportive services.
• Map out next steps for accessing services to achieve ongoing success.
• Document and track patient interactions per AAC, ADHS/DBHS, CARF, and Community Bridges policies.
• Work as part of a multi-disciplinary team with internal and external stakeholders.
