Job summary

CommuniCare Rehab is seeking a Physical Therapist to join the in-house rehab team at Sellersburg Healthcare Center in Sellersburg, Indiana. This full-time position focuses on direct patient therapy, care planning, and collaborative care with the interdisciplinary team.

You will evaluate residents, develop and supervise plans of care, lead staff education, and participate in family training while providing supervision for PTAs.

Qualifications

• Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen residents’ medical charts and physician-ordered evaluations to develop a treatment plan.
• Implement treatment plans through direct therapy and educate the plan of care to all team members; supervise PTAs as needed.
• Attend and lead interdisciplinary meetings to relay findings and recommendations; provide staff and family training when appropriate.
