In this pivotal role, you'll be the face of Diversicare across three of our centers in Birmingham, playing a key part in helping patients and their families navigate their care journey. You’ll collaborate with center teams, build strong relationships with medical professionals, and drive our mission to provide exceptional healthcare.

What you’ll do:

• Be a Transition Champion: Conduct thorough clinical assessments to ensure appropriate and seamless transitions for patients into our care.
• Drive Excellence: Work closely with center teams to achieve and exceed key admission goals, utilizing your knowledge of our centers' unique strengths.
• Build Connections: Cultivate and maintain strong relationships with hospitals, physicians, and other healthcare providers in the Birmingham area.
• Be an Advocate: Champion the highest quality of care and ensure an outstanding customer experience for every patient and family.
• Communicate with Impact: Effectively document and communicate, ensuring clarity and collaboration across all stakeholders.

What you’ll bring:

• Clinical Expertise: RN preferred; LPN, LVN, or Physical/Occupational/Speech or Respiratory Therapist with a current, active license in good standing is required.
• Experience: 1-2 years of experience in skilled nursing or long-term care is essential, with 1-2 years in sales, marketing, or admissions a plus!
• Relationship Building Skills: Established connections with Birmingham-area hospitals and physicians are a strong advantage.
• Strong Assessment Skills: Proven ability to conduct thorough medical assessments and maintain HIPAA compliance.
