Job summary

CommuniCare Rehab, a member of the CommuniCare Family of Companies, seeks a Speech Language Pathologist for a full-time split opening at Evergreen Crossing and Allison Pointe Healthcare Center in Indianapolis. The SLP will evaluate, plan and direct treatment to improve communication, swallowing, and cognition for residents.

The role emphasizes interdisciplinary collaboration, family education, and participation in care conferences. Requires a degree in SLP and state licensure (active or pending).

Qualifications

• Must have a Bachelor's and/or a Master's in Speech-Language Pathology from an accredited program.
• Hold a valid or pending State Speech-Language Pathology License.

Responsibilities

• Evaluate, review, and screen residents' medical charts and develop a treatment plan.
• Plan, direct, or conduct rehabilitative programs for communication, swallowing, and cognition.
• Instruct residents, families, and caregivers to monitor speech and swallowing techniques.
• Attend interdisciplinary meetings and relay clinical findings to the rehab team.
