Job summary

Encore at Foulk in Wilmington, DE is seeking a Full Time Physical Therapist Assistant to support patients across Parkside, Foulk, and Wilmington under the supervision of a Licensed Physical Therapist.

You will provide a range of physical therapy treatments, document progress, and collaborate with the rehab team to implement individualized plans of care.

Candidates should have DE licensure, CPR certification, and strong communication skills; NetHealth documentation experience is a plus.

Qualifications

• Graduate of an accredited PTA program.
• Current DE state licensure.
• CPR Certification.
• Excellent verbal, written, and interpersonal communication skills.
• Proficient with therapy documentation (NetHealth).
• Must pass a medical and physical examination.

Responsibilities

• Attends and contributes to patient care and meetings as a representative of physical therapy.
• Ensure effective and timely delivery of rehab services per plan of care.
• Collaborate with rehabilitation team to implement patient-specific programs.
• Assist Physical Therapist during tests, evaluations, and procedures.
• Instruct patients and caregivers in therapy techniques under supervision.
• Maintain timely documentation for all patients treated.
• Identify and report equipment needs and repairs.
