St. John’s Community Health is hiring a Community Health Worker Floater to cover LA County across various programs. The role supports grant-funded staff on leave, conducts outreach, provides health education, and links patients to services.

The CHW will engage with diverse communities, coordinate care, and help address social determinants of health. Requires a Bachelor's degree or equivalent and at least two years of relevant experience, with EHR proficiency and the ability to travel between

Qualifikationen

• Bachelor’s degree or equivalent experience.
• Two years of relevant experience.
• Knowledgeable with Electronic Health Records (EHR).
• Flexibility to work evenings and weekends as needed.
• Reliable transportation and ability to travel between clinics.
• Maintains professional appearance when interacting with patients, staff and physicians.
• Ability to cope in a fast-paced environment.
• Maintain certifications as required by job function or law and provide verification when requested.
• Community Health Worker licensure (CHW).
• Substance Use Disorder Counselor licensure (Registered/Certified).
• Peer Support Specialist.
• Domestic Violence Counselor.

Aufgaben

• Outreach and Engagement to inform community members about St. John’s services.
• Health Education on conditions and services; educate families and communities.
• Screening and Linkage to Care with warm handoffs to services.
• Assessment and Service Planning using tools to address SDOH.
• Case Management, Care Coordination, and System Navigation for referrals.
• Peer Support, Education, and Advocacy for patients.
• Training staff and partners on health conditions and SDOH.
• Team and Organizational responsibilities and process improvements.
• Grant funded duties as applicable.
