St. John’s Community Health is seeking a Community Health Worker Floater to support grant programs across Los Angeles County. This regular full‑time position fills in for leaves and absences, connecting patients to needed services and ensuring continuity of care.

The role requires a Bachelor’s degree or equivalent experience, 2 years of relevant work, and flexibility to work evenings/weekends. EHR proficiency and travel between clinics are essential.

Qualificações

• Bachelor’s degree or equivalent experience.
• Two years of relevant experience.
• Knowledgeable with EHR (Electronic Health Record).
• Flexibility to work evenings and weekends for events.
• Reliable transportation and ability to travel between clinics.
• Maintains professional appearance with patients and staff.
• Able to cope in a fast-paced environment.
• Maintain required certifications as needed by role or law.

Responsabilidades

• Outreach and Engagement – inform and educate communities about St. John’s services and engage individuals and families.
• Health Education – provide education on health conditions and services to individuals and communities.
• Screening and Linkage to Care – conduct brief screenings and provide referrals with warm handoffs.
• Assessment and Service Planning – assess needs and document service plans and progress.
• Case Management and Care Coordination – link to services and ensure communication among providers.
• Peer Support and Advocacy – provide lay counseling and assist with transportation and navigation.
• Training – develop and deliver trainings on health conditions and SDOH to staff and partners.
• Team/Organization Responsibilities – support team goals and uphold Mission, Vision, Values.
• Additional Duties – as assigned.
