Advocate Health in Winston-Salem, NC is seeking an ambulatory infusion pharmacist to oversee patient care across nine clinic locations. You will provide counseling, evaluate therapy regimens, and ensure safe medication use in a fast-paced ambulatory setting.

The role requires a Pharm.D. or B.S. in Pharmacy, NC license, and immunization certification; leadership and teaching experience are valued for mentorship of trainees.

Formación

• Graduate of an accredited School of Pharmacy with a B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.) is required.
• Current state pharmacist license in good standing through North Carolina Board of Pharmacy.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.

Responsabilidades

• Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
• Counsel patients and caregivers on medication therapy, adverse effects, compliance, and administration.
• Design, monitor, and evaluate patient-specific pharmacotherapeutic regimens and document in the appropriate format.
• Review and interpret medication orders and prescriptions accurately.
• Participate in medical emergency management when applicable.
• Support medication safety and quality improvement activities and report events appropriately.
• Collaborate with the healthcare team on formulary adherence and medication-use processes.
